Practical variations
- Start by finding how individual items in a system can fail, and then build an FTA diagram bottom-up, to show how they might act in combinations to cause an overall system failure.
- Write the failures on 3" x 5" cards or adhesive memo notes, so they can be rearranged into the best order on the diagram.
- Make symbols easier to visually differentiate by writing 'AND' or 'OR' in them, as appropriate.
